Referral Details

There are several ways to access the Manitouwadge Diabetes Health & Wellness Program:   Physician’s Referral Your doctor will complete a referral letter and fax or post mail it to the Manitouwadge Diabetes Health & Wellness Program.  Once we receive the referral you will be contacted by telephone to arrange for an appointment. A mail-out package will be sent to you. Please feel free to take advantage of our Self Assessment Questionnaire  (see link below)   Self Referral You may refer yourself to the Manitouwadge Diabetes Health & Wellness Program. Please refer to the Contact Us page for information on reaching us.  If calling and our Answering Machine greets you, please be sure to leave your name and phone number. When you are contacted you will be given an appointment time and we will send an Information Package to you through the mail. Please feel free to take advantage of our Self Assessment Questionnaire  (see link below)   Caregiver or other health care provider referral Your caregiver or health care provider can telephone our office to make the referral. They will need your full name and telephone number. We will then contact you directly to arrange an appointment. At that time a Mail Out Package will be sent to you.  If you are not able to physically come to the office your family member or other caregiver can schedule an appointment to attend on your behalf.
